Disability Care

At “Care About You” we have extensive experience in providing the highest quality person-centred care to both adults and children with disabilities.

Our comprehensive individual needs assessment is carried out to gain an understanding of the person’s abilities, difficulties, medical conditions, allergies, likes and dislikes and wishes and goals.

This information combined with risk assessments forms the foundation of our person-centred support plan.

Risk assessments are not intended to limit the opportunities for the individual but to find a safe way for them to do the things they want to do. Risk assessment are carried out in collaboration with the individual and family members as required.

Care Support Plans are reviewed and updated on a regular basis to ensure that we enhance the quality of a person’s life and to help them live independent lives to the fullest extent possible in their home and within their community.

Adult Services:

At “Care About You” we have experience in providing care and support for adults with a wide range of disabilities including:

We believe in the importance of a Holistic Approach to care and support and we achieve this through Person-Centred Planning.

We carry out an in-depth assessment to establish what you can do for yourself and what they need help with.
In conjunction with you and your family (if appropriate) we develop a Holistic care and support plan.

All activities are risk assessed to ensure your safety. Risk assessments are about finding safe ways for you to take part in activities that others take for granted.

The services we provide to adults with disabilities are as follows:

  1. Community Integration – At “Care About You” we are aware that unfortunately many people with disabilities experience social isolation for a variety of reasons. We can assist you to have full participation in your local community. We can support you to have a valued social role including assisting you to access services and recreational facilities.  
  1. Personal Care and Support – At “Care About You” we are experienced at assisting individuals with all aspects of personal care including assisting with bathing/showering/bed bath, toileting, incontinence wear, catheter/convene assistance, feeding, dressing.
  1. Home Care Services – At “Care About You” we can provide assistance with household chores and daily living activities.  These activities may include:

  • Laundry;
  • Preparation Of Meals;
  • Helping With Household Pets;
  • Light Housework;
  • Companionship;
  • Shopping;
  • Prompting Medication
  • Ordering Prescriptions/Medication;
  • Collecting Medication. 

  1. Home Care Services – At “Care About You” we have extensive experience in assisting people with physical or intellectual disabilities or mental health difficulties to lead an independent and meaningful life. We have experience in supporting young people who are leaving residential or foster care placements to adapt to independent living. This includes teaching and empowering people to:
  • Learn To Cook;
  • Do Laundry;
  • Do Shopping;
  • Learn About Money Management And Budgeting;
  • Do Housework;
  • Pursue Further Education;
  • Seek Employment;
  • Pursue Social Interests And Hobbies.

Our Holistic Approach is designed to assist your child to reach their full potential in the following areas:

  1. Personal Care and Support – We can assist with all aspects of personal care, peg feeding and support for all disabilities.
  1. Physical Care and Support – We can incorporate programs that have been designed for your child by the occupational therapist or physiotherapist to support the child’s physical development, gross and fine motor skills.
  1. Intellectual Care and Support – We have extensive experience in supporting children with intellectual disabilities. We encourage children to reach their own potential and we value their abilities and offer support and encouragement and celebrate their successes.
  1. Language – Although some children are non-verbal, we do not believe that they cannot communicate. Our staff are committed to enabling children to communicate and make choices using communication devices such as PECS. We also assist with Language programs designed by speech and language therapists.
  1. Emotional Care and Support – At “Care About You” we are aware of the levels of emotional support that some children with disabilities require. We endeavor to understand how frustration can result in behavior that challenges and we work with behavior plans and offer encouragement and support for children.
  1. Social Support – At “Care About You” we believe that Social Integration and Social Inclusion are vital for the growth and development and self-esteem of all children. Through effective care planning and support we can ensure that your child has the opportunity to access their local community and take part in enjoyable social activities.
